New Customer Activation Request

In order to open your account, please provide the information below as completely as possible.  If you need assistance, please contact your nearest Customer Service Center. You may submit this information electronically, or print and fax the completed form to 832.813.4440.  

Also, your sales tax information should be faxed to 832.813.4440 at your earliest convenience.  If you do not provide your state sales tax exemption number or certificate, the appropriate tax will be applied to your billing invoice.
